Am I required to submit my vaccination status now?

Submission of the Parkland College COVID-19 Vaccine Status Form is required of all students. Through submission of this form, a student can make any one of five choices to the question below. 

This form asks students to respond to the following:

Please indicate your COVID-19 vaccine status:

    • I have completed my original vaccine series (2 doses of Pfizer/Moderna or 1 does of J&J) and received a COVID-19 booster.
    • I have received my original vaccine series (2 doses of Pfizer/Moderna or 1 dose of J&J) but have not received a COVID-19 booster.
    • I have received one dose of the original Pfizer/Moderna vaccine series but have not completed it.
    • I am not vaccinated for COVID-19.
    • I am not willing to share my vaccination status.

Students who wish to submit their vaccination records for building access clearance should upload a copy/picture of their record using this Vaccine Status Form. This is the only means of submission. Do not email a copy/picture of your COVID vaccination or other vaccination record to anyone, including college personnel. Please do not email anyone a copy/picture or other vaccination record.

Why should I submit my vaccination status?

In order to access any Parkland facility without having to have a negative COVID test weekly. 

What if I don't want to share my vaccination status with Parkland College?

You indicate on the form that you are not willing to share your vaccination status with us. In that case, you will have to have a weekly negative COVID test in order to access any Parkland facility. 

What forms of vaccination verification are accepted?

The primary verification instrument for vaccination status is a copy/photo of your official vaccination card.

What do I do if I don't have my vaccination card anymore?

Please visit the IDPH Vax Verification Portal. COVID vaccination status can be verified with documentation from this site. You may be able to get a copy of the IDPH vaccination verification from your local public health department office as well. 

What do I do if I have had a change in my vaccination status?

Submit updates to your vaccination status after your initial submission through the Vaccine Card Submission Form

Who will Parkland share my personal health information with?

Parkland only uses the information to determine who is eligible for access to its facilities and to determine isolation and quarantine requirements and for its contact tracing purposes. It does not share the information with any faculty or staff that are not part of its COVID teams. The college does not share vaccination information with anyone outside of the college. 

If I submit verification of my fully vaccinated status, will I still have to test weekly?

No. The easiest way to access Parkland's facilities and have minimal obligations for quarantine requirements in the case of an exposure to COVID-19 is to be fully vaccinated. 

How can I get vaccinated?

For more information from the Center for Disease Control (CDC) on COVID vaccination and to find the nearest vaccination options for you, visit vaccines.gov.

What if I am a Community Education or Adult Education student that does not have a Parkland email address and thus cannot submit the COVID-19 Vaccine Status Form listed above?

Please use the COVID-19 Vaccine Status- Students Without Parkland Email Addresses Form to submit your vaccination status. Access to this form does not require a Parkland Single Sign On authentication or a Parkland email. 

What are my options if I have classes on-campus and refuse to share vaccination status or test as required?

You will not be able to attend an on-campus or hybrid class if you are not vaccinated or are not willing to test as needed. 

Email MTrame@parkland.edu no later than 5 pm on Friday, January 14, 2022 to receive more information on options which may allow you to continue your coursework or may provide eligibility for withdrawal or for a drop without record and refund. Not all students will be eligible for a refund and a drop without record. An approved drop with no record, or withdrawal, while receiving any financial aid, including CARES funds, may result in a reduction, or cancellation, of financial aid awards.

Do I need to disclose my vaccination status?

You are not required to disclose your vaccination status. If you do not disclose your vaccination status, then you will be required to submit to the weekly testing requirement in order to enter Parkland facilities.

What do I do if I have had a change in my vaccination status?

What if I am an employee that doesn't have a Parkland email address so I cannot use the employee submission process above?

Please submit your vaccination status to Parkland College Human Resources Department using the COVID-19 Vaccine Status Employee Form. It will not require a Parkland Single Sign On authentication or a Parkland email in order to access the form. 

What if I am an employee of a contractor that Parkland College has contracted to work within the college facilities?

Please submit your vaccination status to Parkland College Human Resources Department using the COVID-19 Vaccine Status- Contractors Form
