Submission of the Parkland College COVID-19 Vaccine Status Form is required of all students. Through submission of this form, a student can make any one of five choices to the question below.

Students who wish to submit their vaccination records for building access clearance should upload a copy/picture of their record using this Vaccine Status Form. This is the only means of submission. Do not email a copy/picture of your COVID vaccination or other vaccination record to anyone, including college personnel. Please do not email anyone a copy/picture or other vaccination record.
In order to access any Parkland facility without having to have a negative COVID test weekly.
You indicate on the form that you are not willing to share your vaccination status with us. In that case, you will have to have a weekly negative COVID test in order to access any Parkland facility.
The primary verification instrument for vaccination status is a copy/photo of your official vaccination card.
Please visit the IDPH Vax Verification Portal. COVID vaccination status can be verified with documentation from this site. You may be able to get a copy of the IDPH vaccination verification from your local public health department office as well.
Submit updates to your vaccination status after your initial submission through the Vaccine Card Submission Form.
Parkland only uses the information to determine who is eligible for access to its facilities and to determine isolation and quarantine requirements and for its contact tracing purposes. It does not share the information with any faculty or staff that are not part of its COVID teams. The college does not share vaccination information with anyone outside of the college.
No. The easiest way to access Parkland's facilities and have minimal obligations for quarantine requirements in the case of an exposure to COVID-19 is to be fully vaccinated.

Please use the COVID-19 Vaccine Status- Students Without Parkland Email Addresses Form to submit your vaccination status. Access to this form does not require a Parkland Single Sign On authentication or a Parkland email.
You will not be able to attend an on-campus or hybrid class if you are not vaccinated or are not willing to test as needed.
Email MTrame@parkland.edu no later than 5 pm on Friday, January 14, 2022 to receive more information on options which may allow you to continue your coursework or may provide eligibility for withdrawal or for a drop without record and refund. Not all students will be eligible for a refund and a drop without record. An approved drop with no record, or withdrawal, while receiving any financial aid, including CARES funds, may result in a reduction, or cancellation, of financial aid awards.
