Your Rights and Responsibilities as a Patient
    in the Parkland College Dental Hygiene Clinic

1. You have the right to considerate and respectful care that meets the standard of care in the profession. Your responsibility is to give your student dental hygienist and the Parkland staff members the same respect and consideration.

2. You have the right to receive complete and current information regarding the treatment and prognosis of your dental condition. Your responsibility is to provide the student dental hygienist with accurate history of past and present dental and medical conditions so that he/she and the professional staff can make the best decisions about your health.

3. You have the right to receive adequate information necessary to provide informed consent prior to the start of any procedure or treatment. You will be informed of the recommended treatment, treatment alternatives, the approximate length and continuity of treatment and the expected outcome of your treatment. You will have the option of refusing treatment. You will also be advised of the risk of no treatment.

4. You have the right to be informed in non-clinical terms on information needed in order to make knowledgeable decisions on consent or refusal for treatment. When you cannot understand the information provided, the information will be provided to a family member or another appropriate person.

5. You have the right to privacy concerning your health care. All care received at the Parkland College Dental Hygiene Clinic will be kept strictly confidential, except as required by law.

6. You have the right to know the identity and professional status of the student dental hygienist and the professional staff. Introductions of the individuals responsible for your care and their professional status in the clinic will be provided prior to examination and treatment by these individuals.

7. You have the right to know the charge for the services provided by the dental hygiene clinic. Your responsibility is to pay the fee unless you are exempted from this fee.
