- Fee is $10 for all appointments necessary within a 6 month period.
- Free for children under eleven.
- Free for adults over sixty-five, Medicaid or Public Aid recipients.
- Payment by check is preferred the appointment. We cannot process credit or debit cards.
- Sorry, no refund in the event of treatment not completed.
